Don't Forget Box 32

Beginning April 1, there was a new rule for filling out the CMS-1500 form for Medicare Claims. Although box 32 says you are to fill it out with "Name and address of facility where services were rendered (if other than home or office)," in fact, unless you performed the service at the patient's home, you are required to write in your office's physical address (Post Office boxes won't do) even though CMS already has it. Just writing in "doctor's office" will no longer suffice. Psychiatrists have been calling the Managed Care Help Line to report that claims are being rejected because the office address was not listed in box 32. If you get a claim back and don't know what is wrong with it, check to make sure box 32 has an address in it. {blacksquare}
